    Getting Started: It’s so Simple

    Download the Health Questionnaire Form from this website. Fill the out and mail it with a check/money order for the appropriate amount. The Health Questionnaire will provide sufficient information so we can know with reasonable certainty what your body chemistry pattern is. With this knowledge we are able to understand why you have the symptoms that bothers you.

    After the evaluation is completed, you will receive the following:
    • Personalized recommendation sheets
    • An audio cassette tape (optional)
    • A personal telephone conference
    To use this service, fill out the application and mail it with the fee. This covers all consultation with us for 3 months, then $10 each month thereafter.
